Today, 26 February 2019, the inquest began at Avon Coroners Court into the deaths of five people resulting from the accident in September 2017.  At just before 2.30pm on 16 September 2017, a Mercedes box van travelling southbound went through the central reservation and was in collision with two cars on the northbound carriageway. The inquest has heard that a front tyre on the van had been recut following an MOT advisory notice, and it is thought that tyre blew out on the motorway leading to the accident. Richard Evans, 66, his wife Elaine, 62, and Elaine's mother Audrey Hodge, 84, all from Liverpool, were killed in one car during the accident. Adrian Beaumont, 46, from Bristol in the other car was also killed in the crash. Adrian's partner, paediatrician Dr Rebecca Mitchell, 42, and their two children, aged 12 and 10 at the time, were pulled from their burning car. Counsell was in charge of a firework display for Taunton Rugby Club that took place at the time of the crash.  09 December 2013.<br />

Pytch owner Johnny Palmer will use the plane fuselage for an eco-friendly office and event space for the business. The plane as used by Japan Airlines before going into private ownership in the 1970s, sold for £50million when new.
